PCN Health Coaching and Social Prescribing Team

Health Coaching and Social Prescribing is an innovative approach in healthcare that connects people with non-medical support to improve their health and well-being. It recognises that many factors—such as loneliness, stress, financial concerns, or long-term conditions—affect overall health, and sometimes, the best "prescription" isn’t medication but community-based support.

PCN Operations Team

The Operations Team plays a vital role in managing and developing The Rural Practice Network. Fortnightly Operations Group meetings bring together Practice Managers, the Operations Manager, the Business Manager, and the Clinical Director to discuss key financial, operational, and clinical matters. Their decisions and discussions feed into the bi-monthly Full Steering Group, which includes the wider Operations Team. This structure ensures a collaborative approach to financial planning, clinical decision-making, and strategic service improvements, supporting the effective running of the network and enhancing patient care.

Complex Care Team

The Somerset Complex Care Team operates across the, delivering joined-up care for patients with complex needs.

What They Do

The team takes referrals from across the healthcare system, providing:

  • Complex case management
  • Service co-ordination and care planning
  • High-quality communication across health and social care

How It Works

  • Multi-disciplinary team (MDT) huddles are held weekly, bringing together primary, secondary, and community care professionals, alongside social care and voluntary sector teams.
  • The team works closely with GPs, community teams, and hospitals to ensure personalised care planning and crisis prevention.
  • A co-ordinated approach helps streamline care, reduce duplication, and improve patient outcomes.
  • The team also supports training for medical trainees and advanced practitioners in complex care management.

Impact

  • Improved patient care through a co-ordinated, person-centred approach.
  • Reduced hospital admissions and better continuity of care.
  • Stronger collaboration between health, social care, and voluntary sectors.
